National Physical Therapy Assistants Examinations - FAQ Sample Questions

Understanding functional anatomy is important when treating fractures. Which kind of nerve damage can occur from a fracture to the middle of shaft of the humerus?

Correct Answer:
radial nerve palsy.

understanding the functional anatomy of the upper limb is crucial when diagnosing and treating fractures, especially in areas rich with neurovascular structures such as the humerus. the humerus is the long bone in the upper arm, extending from the shoulder to the elbow. its position and relationship with nearby nerves make certain areas particularly susceptible to nerve injuries when fractured.

one common complication from a fracture to the middle of the shaft of the humerus is radial nerve palsy. the radial nerve travels along the posterior aspect of the humerus, in a groove known as the radial groove. due to its proximity to the bone, the radial nerve is at risk of being compressed or damaged when a fracture occurs in the mid-shaft of the humerus. this can lead to radial nerve palsy, characterized by symptoms such as wrist drop, difficulty in extending the wrist and fingers, and numbness in part of the hand.

the radial nerve's primary function is to innervate the posterior compartment of the forearm, which is crucial for extending the wrist and fingers. it also provides sensory innervation to the back of the hand. damage to this nerve can severely impact the function of the hand and forearm, making it a significant concern in mid-shaft humeral fractures.

other nerves, such as the axillary nerve, peroneal nerve, and femoral nerve, are less likely to be affected by a mid-shaft humeral fracture. the axillary nerve runs more proximally near the shoulder and innervates the deltoid and teres minor muscles, as well as providing sensation over part of the shoulder. the peroneal and femoral nerves, on the other hand, are located in the lower limb and therefore not relevant to fractures of the humerus.

in summary, when treating fractures of the middle of the shaft of the humerus, it is essential to consider the potential for radial nerve palsy due to the anatomical course of the radial nerve along the radial groove of the humerus. proper assessment and management are necessary to prevent long-term dysfunction and facilitate recovery.
