in-toeing in toddlers, where the feet turn inward while walking, is a relatively common developmental observation. this condition can stem from various anatomical variations including medial tibial torsion, metatarsus adductus, adduction of the great toe, and femoral anteversion. each of these conditions contributes differently to the in-toeing posture seen in young children.

**adducted great toe**, or a condition where the big toe is turned towards the other toes, can also contribute to in-toeing. while this is less commonly discussed compared to the other conditions, it can still be a factor in how a child's walking pattern develops.

in conclusion, among the options listed, legg-calve-perthes disease is the exception when considering potential concerns for an 18-month-old child with in-toeing. this disease generally affects older children and would not be a typical cause or concern for the inward turning of the feet in a toddler. the other conditions mentioned are more relevant to the in-toeing seen in young children.
