Osmotic diuretics like mannitol work by increasing the osmolarity of the blood and the renal tubular fluid. Mannitol, which is a sugar alcohol, is filtered through the kidneys but is not reabsorbed. By remaining in the renal tubule, it holds onto water through osmotic forces, which prevents the water from being reabsorbed into the bloodstream. Consequently, this increase in osmolarity in the renal tubule leads to an increase in the volume of urine produced.
The primary effect of increasing the osmolarity and fluid levels in the renal tubules is the reduction of water reabsorption back into the bloodstream. This action is crucial in clinical scenarios where reducing fluid volume in the body is necessary, such as in cases of cerebral edema (swelling of the brain) or acute renal failure, where mannitol can help flush out toxins and alleviate swelling.
It's important to differentiate mannitol from other types of diuretics. For example, loop diuretics like Bumex (bumetanide) and Lasix (furosemide) function by inhibiting the sodium-potassium-chloride cotransporter in the loop of Henle in the nephron, which leads to a different mechanism of reducing fluid retention. Potassium-sparing diuretics, such as Midamor (amiloride) and Dyrenium (triamterene), work by either blocking sodium channels or aldosterone receptors in the distal tubule and collecting duct, which helps the body retain potassium while excreting sodium and water.
